About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations for health plan members in the Danville-Martinsville, Virginia area. Physicians visit people in their homes to build trusted relationships, help bridge gaps in care, and connect members with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, when ordered by a member’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Physicians use an iPad and a clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role requires local travel and travel within licensed states as needed, as well as virtual visits when needed. The position is full time, with 40 anticipated weekly hours.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Provide in-home health evaluations and build trusted relationships with health plan members.
• Review members’ medications and medical histories and conduct physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Help connect members with appropriate care and support improved health outcomes.
