In-Home Nurse Practitioner or Physician Assistant Full Time - Danville-Martinsville, VA
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and a clinical workflow and are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The role may also include virtual visits and travel locally and within licensed states as needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. The position is full time, with 40 anticipated weekly hours. The stated pay range is $87,035 to $187,460.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to perform duties comfortably and safely in members’ homes, which may contain household pets, secondhand smoke, or other potential airborne allergens or irritants.

Responsibilities
- Conduct In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic tests when ordered by a member’s health plan, such as diabetic eye exams, spirometry, and bone density screening.
- Build trusted relationships with health plan members and help connect them with care suited to their needs.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Use an iPad and the clinical workflow for evaluations.
- Provide health education and answer members’ health questions.
