In-Home Nurse Practitioner or Physician Assistant Full Time - Roanoke, VA
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in the Roanoke, Virginia area. Clinicians visit health plan members in their homes to build trusted relationships, help bridge gaps in care, and provide flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include time for one-on-one engagement and answering health questions, a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. The role includes local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; candidates must be able to perform their duties comfortably and safely in these conditions. Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to perform duties safely and comfortably in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities
- Conduct In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
- Engage with members one-on-one, answer their health questions, and help connect them with care suited to their needs.
- Provide additional diagnostic tests when ordered by the member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
- Use an iPad and the clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
