In-Home Nurse Practitioner or Physician Assistant Full Time - Danville-Martinsville, VA
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members. Clinicians conduct visits of about 45 minutes in members’ homes, using the time to build trusted relationships, answer health questions, and help bridge gaps in care. Evaluations include reviewing medications and medical history and performing a physical evaluation. Additional diagnostic tests may be performed when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. The clinician must be able to perform duties comfortably and safely in these environments. Local and in-state travel is required as needed, and virtual visits may also be conducted. The position is full-time, with 40 anticipated weekly hours. The typical pay range is $87,035.00–$187,460.00. The actual base salary offer depends on factors such as experience, education, and geography.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations and build trusted relationships with health plan members.
- Review members’ medications and medical history and perform a physical evaluation.
- Perform additional diagnostic tests when ordered by the member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
- Use an iPad and clinical workflow to complete evaluations.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services, including Diagnostic & Preventive Services products as indicated.
