About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations and help bridge gaps in care. Physicians visit health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, and personalized evaluations. A visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Physicians may also conduct virtual visits as needed and travel locally and within licensed states as needed. The role is part-time, per diem, with an anticipated 12 weekly hours and an hourly base rate of $105, plus possible visit-based and other incentives. Per diem employees receive malpractice coverage for Signify Health-related activities, supplies, and other perks.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to safely and comfortably perform duties in members’ homes, which may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Provide in-home health evaluations and engage with members in their homes.
• Review each member’s medication and medical history and conduct a physical evaluation.
• Perform additional diagnostic tests when ordered by the member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Help connect members with appropriate care and support efforts to improve health outcomes.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
