About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians engage with people in their homes, build trusted relationships, and provide flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by a person’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Clinicians must be able to perform their duties safely and comfortably in these settings. The role may also involve local travel and travel within licensed states, as needed, and virtual visits as needed.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a person’s health plan, such as diabetic eye exams, spirometry, or bone density screening.
• Use an iPad and the clinical workflow to complete evaluations.
• Build trusted relationships with health plan members and help connect them with care suited to their needs.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
