About The Position

Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations and help bridge gaps in care. Visits take about 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and clinical workflow and are not required to prescribe medication, order lab tests, or alter a person’s current treatment regimen. The role involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. The position is part time, with 29.5 anticipated weekly hours.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations and build trusted relationships with health plan members.
• Review members’ medication and medical history and perform a physical evaluation.
• Answer health questions and conduct additional diagnostic tests when ordered by the member’s health plan.
• Use an iPad and the clinical workflow for evaluations.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, and bone density screening.
• Fulfill the requirements of the specified clinician role and contribute to efforts to improve health outcomes and connect people with appropriate care.
