About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations for health plan members. Visits last approximately 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and a clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The role may include virtual visits and requires flexible local travel and travel within licensed states as needed. The primary workplace is members’ private residences; home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. The position is part time, with approximately 12 anticipated weekly hours.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to safely and comfortably perform duties in private homes that may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations and build trusted relationships with health plan members.
• Review members’ medication and medical history and perform physical evaluations.
• Answer members’ health questions and help connect them with appropriate care.
• Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, or bone density screening.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated and fulfill the requirements of the clinician role.
