About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner to provide in-home health evaluations and help bridge gaps in care. The role involves engaging with health plan members in their homes and building trusted relationships to support better health outcomes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen.

The position involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions.

This position is part-time, with 29.5 anticipated weekly hours. Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer health questions and help members connect with care suited to their needs.
• Perform additional diagnostic and preventive services when indicated, such as spirometry and diabetic eye exams.
• Use an iPad and clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
