About The Position
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, and personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen. Virtual visits may also be conducted as needed.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- Board certification.

Responsibilities
- Conduct in-home health evaluations and build trusted relationships with health plan members.
- Review members’ medication and medical history and perform physical evaluations.
- Perform additional diagnostic and preventive services as indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Fulfill the requirements of the specified clinician role and contribute to efforts to improve health outcomes and connect members to appropriate care.
