About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians meet with health plan members in their homes to build trusted relationships and provide flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include time for one-on-one connection and questions, a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter members’ current treatment regimens. Virtual visits may also be conducted as needed. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions. The role requires flexibility to travel locally and within licensed states as needed.

Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Answer health questions and connect with members one-on-one during visits.
Provide additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams, spirometry, and bone density screening.
Conduct virtual visits as needed.
Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
Travel locally and within licensed states as needed.
Help members connect with appropriate care for their individual needs.
