About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in Grand Island, Nebraska. The clinician visits health plan members in their homes, builds trusted relationships, helps identify gaps in care, and connects people with appropriate care for their individual needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order laboratory tests, or change a person’s current treatment regimen.

The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens and irritants. Clinicians must be able to work comfortably and safely in these varied environments. The role also includes local travel and travel within licensed states as needed, as well as virtual visits when needed.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to safely and comfortably perform duties in members’ homes, which may contain pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Build trusted relationships with health plan members and help connect them with appropriate care.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
