About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations to health plan members. The role focuses on building trusted relationships, helping people understand their health, and connecting them to appropriate care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The physician must be able to safely and comfortably perform duties in these settings. The role includes local travel and travel within licensed states as needed, as well as virtual visits when needed. The listed primary coverage area is Marquette, including Alger, Baraga, and Marquette counties.

Per diem employees receive an hourly base rate with an additional incentive structure, malpractice insurance coverage for Signify Health-related activities, supplies, and other perks. The position is part-time and anticipates 12 weekly hours. The listed base pay is $105.00 per hour. Benefits for this part-time role may include dental and vision coverage, wellness resources, employee discounts, access to certain voluntary benefits, and other programs, depending on hours worked and eligibility.

Requirements

• Hold active, unrestricted physician license(s) in the coverage area(s).
• Be able to safely and comfortably perform duties in members’ homes, which may contain pets, secondhand smoke, and other airborne allergens or irritants.
• Be able to cover Marquette and the counties of Alger, Baraga, and Marquette.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services when indicated, such as diabetic eye exams and spirometry.
• Use an iPad and the clinical workflow for each evaluation.
• Help connect members to appropriate care and support health outcomes.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
