About The Position

Signify Health, part of CVS Health, is seeking a per diem Physician to conduct in-home health evaluations for health plan members. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. The role focuses on building trusted relationships, helping members identify care needs, and contributing to improved health outcomes. Local and in-state travel is required as needed, and virtual visits may also be conducted.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Per diem employees receive an hourly base rate, with additional incentives potentially available, malpractice coverage for Signify Health-related activities, supplies, and other perks. This part-time role may qualify for benefits and programs depending on hours worked and eligibility.

Requirements

• Active, unrestricted medical license(s) in coverage area(s).
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Conduct one-on-one in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow to complete each evaluation.
• Help connect members with appropriate care and support improved health outcomes.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services, including Diagnostic & Preventive Services products as indicated.
