In-Home Clinician NP/PA - Full Time - Colorado Springs, CO
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in the Colorado Springs/El Paso County area. The role focuses on building trusted relationships, helping people improve their health, and connecting them with appropriate care. Visits last about 45 minutes and include a medication and medical history review and a physical evaluation. Additional diagnostic tests, such as diabetic eye exams or spirometry, may be provided when ordered by the member’s health plan. Clinicians use an iPad and clinical workflow. They are not required to prescribe medicine, order lab tests, or change members’ current treatment regimens. The clinician’s primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Local travel and travel within licensed states may be needed, and virtual visits may be conducted as needed.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Able to cover Colorado Springs/El Paso County.
- Able to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities
- Provide in-home health evaluations to health plan members.
- Review members’ medication and medical history and conduct a physical evaluation.
- Provide additional diagnostic and preventive services when indicated, including services such as spirometry and diabetic eye exams.
- Build trusted relationships with members and help connect them with care suited to their needs.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Fulfill the requirements of the clinician role and provide Signify Health services as indicated.
