About The Position

European Service at Home (ESAH), an Illinois in-home services organization, is seeking a committed Care Coordinator for its local office. The organization supports seniors who wish to continue living at home and promotes diversity and quality client service. The role provides responsive, compassionate customer service to senior clients and involves case management and direct supervision of Home Care Aides. The Case Coordinator works with a team to meet office workload and goals, builds client-centered relationships that support continued participation in care services, and provides help with technical questions, applications, and problem-solving. The role also evaluates, plans, coordinates, and manages Home Care Aides; maintains relationships with the Community Care Unit; and keeps client and employee records complete and current. Starting wage is $25 per hour, based on experience.

High school diploma.

Responsibilities

Work in a team environment to accomplish the office’s workload and goals.
Build and foster trusted, client-centered relationships to keep clients engaged in care services based on the Care Program.
Serve as a resource for technical questions, applications, and problem-solving.
Evaluate, plan, coordinate, and manage Home Care Aides to support the interests of clients and the company.
Maintain a good-standing relationship with the Community Care Unit.
Ensure client and employee records are complete and current.
