About The Position

The Housing Social Services Coordinator works at Wesley by the Bay, a senior apartment community operated by United Methodist Communities in Ocean City, New Jersey. Residents live independently, and the coordinator helps them learn about and access health, home care, and community services that support their independence. The role includes building relationships with residents, connecting them with resources, supporting care transitions, maintaining records, and coordinating resident programs. The schedule is full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. Pay is $21.01 to $23.63 per hour, based on experience.

Requirements

• High school diploma or GED.
• Three years of experience serving older adults or people with disabilities, or in social services in a healthcare setting.

Responsibilities

• Build relationships with residents and connect them with available services and resources.
• Stay in contact with residents who may need additional support, including those returning from rehabilitation or hospital stays.
• Assist residents and families with care transitions and available housing and service options.
• Maintain resident records, program documentation, and required reports.
• Coordinate educational, wellness, social, and recreational programs for residents.
• Prepare newsletters and community updates and support resident groups and committees.
• Partner with leadership and team members to support resident eligibility, documentation, and program requirements.
