Hospice RN Clinical Director
About The Position
Hearth Hospice is seeking a Registered Nurse with clinical knowledge and supervisory experience to lead its clinical team. The Clinical Director oversees an interdisciplinary team of RNs, aides, social workers, and chaplains, and works closely with the Program Director, Hospice Physician, and Community Liaisons. The role supports office operations, patient growth, and compliance with Federal, State, ACHC, and Hearth guidelines. The position is based in the Cleveland Office and is scheduled Monday through Friday, 8:00 a.m. to 4:30 p.m., with rotating administrative on-call duty.
Requirements
- Registered Nurse with a current state license to practice in the state where the office is located.
- At least 3 years of nursing experience, including 1 year of full-time direct patient care in hospice, home health, or oncology.
- Demonstrated ability to manage terminal care, including crisis intervention, pain and symptom control, bedside clinical nursing, and family counseling.
- Demonstrated ability to respond supportively and sensitively to staff and patient/family needs.
- Computer proficiency, including typing and experience with various programs and EMR.
- Current CPR certification if required by the state.
- Current driver's license, vehicle insurance, and access to dependable transportation.
Nice To Haves
- Bachelor's degree in nursing or health care preferred.
- At least 1 year of supervisory experience in hospice or home health care preferred.
Responsibilities
- Coordinate and oversee patient services in alignment with hospice program policies.
- Guide and support clinicians and foster continuous improvement in hospice care services.
- Conduct chart audits and oversee medical record maintenance to ensure compliance with regulations, certification standards, and ethical care benchmarks.
- Participate in developing orientation and in-service training for nursing and clinical staff.
- Evaluate clinical staff performance during probationary periods and annually, providing teaching and counseling to improve service delivery.
- Lead interdisciplinary team meetings to refine patient and family care plans, exchange insights, resolve challenges, and support professional collaboration.
