About The Position

Suncrest Hospice aims to redefine hospice care by offering exceptional service and care to patients in the area. This is achieved by increasing staff to patient ratios, which allows for more clinical visits and lower clinician caseloads. Suncrest Hospice is a CHAP certified hospice. We are seeking individuals committed to providing the highest quality of care to patients and their families.

Requirements

  • Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
  • At least 1 year of hospice experience.
  • Ability to work collaboratively within a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Ability to work independently on a day-to-day basis.
  • Understanding of hospice rules and regulations.
  • Time management skills to manage visits, documentation, and meetings.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, valid driver's license, and auto insurance to travel across the service area.

  • Works well within a team environment.
  • Works closely with patients on a 1-1 level but gets strong support from a patient care team.
  • Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals.

Responsibilities

  • Provide care and services for a group of patients.
  • Visit patients in their residences, including private homes, Skilled Nursing Facilities, and assisted living facilities.
  • Serve as a contributing member of the interdisciplinary team.
  • Provide expertise in psych-social assessments, interventions, and knowledge of community resources.
  • Work in a Case Management, Admissions, or Resource role.
