About The Position

Bloom Healthcare is an employee-owned primary care and hospice practice providing high-touch care to vulnerable, homebound patients with chronic conditions. The Hospice Registered Nurse Float provides and coordinates hospice care for patients in the community or wherever they reside. The role supports and guides the interdisciplinary hospice team in addressing patients’ physical, emotional, and spiritual needs; educates patients and families about the disease process; manages symptoms; and offers support through end-of-life care.

Requirements

• Graduate of an accredited nursing school.
• Valid Colorado RN license.
• At least one year of experience in med-surg, emergency, or critical care nursing.
• Strong clinical assessment capability.
• Excellent communication and interpersonal skills.
• Effective problem-solving skills.
• Detail-oriented with strong time management.
• Computer proficiency.

Nice To Haves

• Experience in hospice or oncology.
• Understanding of death, dying, and bereavement processes.
• CHPN certification.

Responsibilities

• Deliver hospice nursing care.
