About The Position
The RN Hospice coordinates and oversees hospice care for assigned patients. The position develops and implements individualized plans of care with the interdisciplinary team, performs assessments, monitors symptoms, manages medications, educates patients and families, and communicates patient progress with clinical leadership, physicians, and the care team. This is a full-time, permanent hire position offering full benefits, competitive compensation, and a bonus structure.
Requirements
- Current Registered Nurse license.
- Current driver's license.
- Current CPR certification.
- Two years of Registered Nurse experience in a clinical care setting.
- One year of full-time hospice experience.
- Reliable transportation and valid automobile liability insurance.
Responsibilities
- Coordinate and oversee all aspects of assigned patients' hospice care.
- Develop and implement individualized plans of care with the interdisciplinary team.
- Perform head-to-toe assessments, monitor symptoms, and manage medications.
- Educate patients and families regarding condition, needs, and care.
- Communicate patient progress with clinical leadership, physicians, and the care team.
