About The Position
Grace Healthcare Services is seeking a Registered Nurse Hospice Liaison Float to work at the RWJ Livingston and Jersey City campuses in a contracted hospital setting. The role combines clinical care, business development, and support for patients and families during transitions in care. The nurse joins the Interdisciplinary Team (IDT) and works a schedule of four 10-hour days, 8:30 a.m. to 6:30 p.m., including weekends at the same hours.
Requirements
- Valid Registered Nurse (RN) license.
- Strong communication and relationship-building skills.
- Compassionate, patient-centered approach.

- Experience in hospice or palliative care.
Responsibilities
- Build and nurture strategic partnerships with new and existing referral sources to facilitate appropriate referrals.
- Identify and assess patients’ psychosocial, spiritual, and physical needs in the inpatient setting.
- Coordinate comprehensive care plans and ensure transitions to the appropriate level of care.
- Conduct pre-discharge hospital visits and offer support and guidance to patients and their families.
