About The Position

The On-Call LVN provides care and services to a group of hospice patients, visiting them where they reside, including private homes, assisted living facilities, and hospitals. The position involves after-hours on-call work on a seven-days/nights-on, seven-days/nights-off schedule. The LVN works closely with patients one-on-one while collaborating with and receiving support from a patient care team.

Requirements

• Current LVN licensure in good standing in the state(s) of practice.
• One year of experience under license.
• Ability to work well within a team and collaborate with the patient care team.
• Ability to work independently on a day-to-day basis.
• Time-management skills to manage visits, documentation, and meetings and to prioritize and triage urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive, with reliable transportation, a valid driver's license, and auto insurance.
• Understanding of hospice rules and regulations.

Responsibilities

• Provide care and services to a group of hospice patients.

• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Manage visits, documentation, and meetings; respond to urgent needs by prioritizing and triaging multiple concerns.
• Communicate by phone, email, and in writing and meet with healthcare professionals.
• Travel across the service area to visit patients.
