About The Position

Suncrest at Brighton/Suncrest Hospice seeks an On-Call LVN committed to providing high-quality care to hospice patients and their families. The role provides care and services to a group of hospice patients, visiting them where they reside, including private homes, assisted living facilities, and hospitals. The position is after-hours on-call and follows a seven-days/nights-on, seven-days/nights-off schedule. Suncrest describes its care model as allocating resources to increase staff-to-patient ratios, increase clinical visits, and lower clinician caseloads. The hospice is Community Health Accreditation Partner (CHAP) certified.

Requirements

• Current LVN licensure in good standing in the state or states of practice.
• One year of experience under license.
• Ability to work independently and as part of a team.
• Time-management skills to manage visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive, with reliable transportation, a valid driver's license, and auto insurance.
• Understanding of hospice rules and regulations.

Responsibilities

• Provide care and services to a group of hospice patients.
• Visit patients in their residences, including private homes, assisted living facilities, and hospitals.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Manage visits, documentation, and meetings, and respond to urgent needs by prioritizing and triaging multiple concerns.
• Communicate by phone, email, and in writing, and meet with healthcare professionals.
• Travel across the service area.
