About The Position

The Hospice Liaison promotes Halifax Health services—including hospice, palliative, and supportive care—in the community through educational in-services. The role serves as a liaison with physicians’ offices, healthcare facilities, hospitals, and community groups, providing education about the continuum of care services offered.

The position establishes and maintains relationships with referral sources, educates medical and non-medical communities about skilled care and end-of-life services, and acts as a link between referral sources and clinical operations. It also supports customer satisfaction and patient outcomes, develops plans for referral sources, identifies barriers to early referral, and documents customer feedback in the Halifax EMS system.

Requirements

• Knowledge of hospice philosophy of care.
• Knowledge of Medicare conditions of participation.
• Knowledge of State of Florida regulations governing hospice.
• Knowledge of hospice operations, sales, and marketing.
• Good written, verbal, and public speaking skills.
• Valid Florida driver license and clear driving record.
• Bachelor’s degree in a related field or higher, or equivalent years of experience.

• Spanish-speaking ability.

Responsibilities

• Establish and maintain working relationships with referral sources, including physicians, hospital case managers, nursing home staff, assisted living facility staff, and other allied healthcare professionals.
• Educate and serve as an ongoing resource for the medical and non-medical community about skilled care and end-of-life care and services offered by Halifax Health-Hospice.
• Educate and serve as an ongoing resource about Halifax Health system services.
• Develop plans of care for referral sources based on referral practices and identify barriers to early referral.
• Develop and maintain a good customer knowledge base.
• Act as a liaison between referral sources and the health system’s clinical operations.
• Provide feedback to support optimal customer satisfaction and patient outcomes.
• Document customer compliments and complaints in the Halifax EMS system.
• Promote Halifax Health hospice, palliative, and supportive care services through educational in-services in the community.
