About The Position

Promotes Halifax Health hospice, palliative, and supportive care services in the community through educational in-services. Serves as a liaison with physicians’ offices, healthcare facilities, hospitals, and community groups, providing education about the continuum of care services. Establishes and maintains relationships with referral sources and acts as a liaison between referral sources and the health system’s clinical operations.

Requirements

• Knowledge of hospice philosophy of care.
• Knowledge of Medicare conditions of participation, Florida state regulations governing hospice, hospice operations, and sales and marketing.
• Good written, verbal, and public speaking skills.
• Possession of a valid Florida driver license and a clear driving record.
• Bachelor’s degree in a related field or higher, or equivalent years of experience.

• Medical sales and marketing experience.
• Spanish-speaking ability.

Responsibilities

• Establish and maintain working relationships with referral sources, including physicians, hospital case managers, nursing home staff, assisted living facility staff, and other allied healthcare professionals.
• Educate and serve as an ongoing resource for the medical and non-medical community about skilled care, end-of-life care, and services offered by Halifax Health-Hospice.
• Educate and serve as an ongoing resource about Halifax Health system services.
• Develop plans of care for referral sources based on referral practices and identify barriers to early referral.
• Develop and maintain a good customer knowledge base.
• Act as a liaison between referral sources and the health system’s clinical operations.
• Provide feedback to support customer satisfaction and patient outcomes.
• Document customer compliments and complaints in the Halifax EMS system.
