About The Position

Full Spectrum Search Group is a full-service healthcare recruiting agency specializing in post-acute leadership and healthcare administration. They are representing a hospice provider seeking a hospital-focused Hospice Liaison to grow hospital referral relationships across the Cleveland, OH metro. This is a relationship-driven growth role for a proven hospice liaison with an established Cleveland hospital book of business. The role is central to expanding hospital-sourced referrals and increasing home-based hospice admissions in the market. The Hospice Liaison will own hospital referral development throughout the Cleveland metro. This experienced sales professional will build and sustain relationships with hospital-based referral partners, generate consistent hospice admissions, and help shift the market's referral mix toward hospital sources and home-based patients. This role is designed for a current hospice liaison or hospital-facing hospice sales representative who can speak clearly to their active hospital accounts, referral-source mix, and monthly admissions production.

Requirements

• Current experience as a Hospice Liaison, Hospice Account Executive, Hospice Business Development Representative, or hospital-facing hospice sales professional in the Cleveland market.
• Established relationships within Cleveland hospital systems and hospital-based referral sources.
• A current book of business that is approximately 90% hospital-sourced.
• Consistent history of producing 10 to 20 hospital-sourced hospice admissions per month.
• Ability to clearly explain monthly admissions volume, referral-source mix, active accounts, and relationship-development approach.
• Demonstrated success building referral flow with hospital discharge planning and care-transition teams.
• Facility-based referral relationships are welcome, provided hospitals are the primary source of admissions.
• Strong knowledge of hospice services, patient eligibility conversations, and the hospital-to-home hospice transition process.

Responsibilities

• Build, maintain, and grow referral relationships with hospital systems and hospital-based referral sources throughout the Cleveland, OH metro.
• Develop productive relationships with discharge planners, case managers, social workers, physicians, and other hospital stakeholders involved in hospice referrals.
• Increase the volume of home-based hospice patients referred through hospital channels.
• Manage a hospital-centered territory and maintain consistent visibility with key referral partners.
• Communicate hospice eligibility, services, and care-transition options in a compliant, clinically appropriate manner.
• Track referral activity, admissions production, and referral-source mix to ensure hospitals remain the core source of business.
• Coordinate closely with hospice operations and clinical teams to support a strong referral experience and timely patient transitions.
• Maintain some facility-based relationships as appropriate, while keeping hospital referrals as the primary focus of the territory.
