About The Position

The Hospice Liaison advances Presbyterian Homes & Services’ mission by building trusted relationships and providing non-clinical hospice education to PHS senior living communities, residents, families, and referral partners. This field-based role supports earlier identification of residents who may benefit from hospice and timely, informed consideration of services while respecting each individual’s freedom of choice. The liaison serves as an education and relationship resource for assigned communities and referral sources. The role works collaboratively with Campus Administrators, Clinical Administrators, Genevive primary care, Optage Home Health, hospice clinical leadership, and the centralized Access and Admissions team. The position reports to the Director of Business Development and covers assigned markets in Minnesota or Eastern Wisconsin. The Hospice Liaison provides non-clinical information and coordinates appropriate follow-up and handoffs, but does not participate in clinical assessments or eligibility decisions. Clinical questions are referred to the registered nurse team, while eligibility and admission decisions are directed through Access and Admissions and the appropriate hospice physician.
