Hospice Liaison (Patient Advocate)

Ovation Hospice of Denver•Denver, CO

About The Position

Ovation Hospice is hiring Patient Advocates to connect patients and families with hospice care. This is a field-based relationship role involving daily community outreach and relationship-building with physicians, hospital discharge teams, skilled nursing facilities, case managers, and other healthcare partners. The Patient Advocate serves as a bridge for families navigating end-of-life care and is responsible for developing a referral territory, tracking performance, and supporting qualified referrals through to admission. The role is not described as transactional sales; it calls for relational skill, professionalism, self-direction, resilience, and alignment with the hospice mission. The position is being hired as a cohort across Ovation Hospice's three markets, with selected candidates sharing a start date, onboarding experience, and compensation structure. The hiring process includes a written application question, phone screen, structured in-person interview, behavioral assessment, and a mandatory 2–3-hour field ride with an Ovation leader.
