Ovation Hospice of Denver•Western Hills, CO
•$70,000 - $90,000•Onsite

About The Position

Ovation Hospice is hiring a field-based Patient Advocate (Hospice Liaison) in Denver, Colorado. The role connects patients and families with hospice care by building relationships with healthcare referral partners and supporting families as they navigate their options. It is a relationship-focused position, not a transactional sales role, requiring professional credibility, compassion, accountability, and the ability to develop a referral territory. The Patient Advocate works in the community with physicians, hospital discharge teams and case managers, skilled nursing facilities, and other healthcare partners. The role also involves explaining hospice benefits to families, generating qualified referrals and admissions, maintaining CRM records, and coordinating with the Administrator, clinical team, and intake coordinators to support smooth hand-offs and patient experience. This position is being hired as part of a cohort across Ovation Hospice’s three markets, with selected candidates sharing a start date, onboarding experience, and compensation structure. The hiring process includes an application question, phone screen, structured in-person interview, behavioral assessment, and a mandatory 2–3-hour field ride. Compensation includes a stated base salary of $70,000–$90,000, a performance-based bonus plan, mileage stipend, and cell phone reimbursement. Benefits include medical, dental, and vision coverage, PTO, and a 401(k) retirement plan.

  • Self-directed approach to planning work, managing a territory, and following up independently.
  • Resilience when facing lost referrals or slower periods.
  • Motivation to track personal performance and meet goals.
  • Genuine alignment with the hospice mission and sensitivity to the importance of helping families access care.
  • Complete the application and prescreen survey; submit a brief response about building a relationship from scratch in a difficult environment.
  • Participate in the required field ride, which lasts 2–3 hours.
