About The Position

The Home Medication History Information Technician works under the direction of the Home Medication Information RN to help provide indirect patient care. The role supports staff during admission or as needed by helping obtain the best possible list of medications a patient takes at home and ensuring the home medication information process is completed according to policy.

The schedule is 11:00 AM–11:00 PM, three days per week, with one 8-hour shift every two weeks (80 total).

Requirements

• High school diploma or GED.
• One year of experience in a retail pharmacy setting.
• Current state Board of Pharmacy registration as a Pharmacy Technician (LPT) or Pharmacy Intern (RPI), required upon hire.

Nice To Haves

• Hospital pharmacy experience.
• IV admixture experience.
• IV chemotherapy preparation experience.
• PTCB certification.

Responsibilities

• Help ensure the home medication information process is completed according to policy.
• Serve as a resource to staff during patient admission or as needed to obtain the best possible list of the patient’s home medications.
