About The Position

Angels of Care Pediatric Home Health is hiring a Home Health Transitional Nurse to assess patients’ post-acute needs in collaboration with the multidisciplinary Halos Home Team and patients’ discharge care providers, ensuring continuity of care when patients are discharged home. The role primarily serves medically fragile infants, children, and adolescents in their homes, following the physician’s active plan of care and guidance from other appropriate personnel. It also provides interim care when staffing gaps occur and helps train field nurses during case transitions. The nurse works with patients, families, caregivers, clinical care managers, discharge providers, support staff, and field nurses. The position requires flexibility to work on call as needed, travel locally, and provide care in patients’ homes. Compensation is $40–$42 per hour. The posting describes office and home work settings, physical and emotional demands, and travel of approximately one hour from the branch office or a further agreed-upon distance.

Requirements

Active PA RN or LPN license.
Active CPR certification.
Strong oral and written communication skills.

Responsibilities

Assess patients’ post-acute needs with the multidisciplinary Halos Home Team and patients’ discharge care providers to support continuity of care when discharging home.
Provide care to infants, children, and adolescents primarily in their homes according to the active plan of care.
Conduct ongoing patient care and assessments.
Administer prescribed medications, treatments, and therapies.
Coordinate patient care.
Educate family members and caregivers on clinical care to enhance positive outcomes.
Use preventative initiatives to protect quality of care.
Provide interim care to patients experiencing staffing gaps due to call-offs, vacations, or staff shortages, with varying acuity and scheduling needs.
Provide one-on-one training to field nurses to support clinical and non-clinical transitions and continuity of care when the Halos Home Team exits a case.
Work with support staff as a liaison and patient advocate on complicated cases.
Maintain positive internal and external customer service relationships.
Exercise necessary cost-control measures and meet productivity requirements.
Communicate effectively, listen, and ask questions when clarification is needed.
Plan and organize work, ensure its completion, and contribute to a team-oriented environment.
Represent the organization professionally, demonstrate initiative, and continually improve processes and relationships.
