About The Position

BAYADA Home Health Care is seeking a compassionate, relationship-focused Transitional Care Manager to support the MGL division in Northern Colorado. The role may be filled by a qualified clinical or non-clinical professional and combines referral management, communication, relationship-building, and transition planning to help clients and families move safely and successfully to care at home. The manager partners with referral sources, patients, families, the marketing team, and BAYADA service-office teams. The position follows a hybrid work model, with office work and approximately 25–50% field time throughout Northern Colorado. Regular business-hours availability is expected, including while working outside the office. Regular on-call coverage is not expected. Training and ramp-up are expected to continue for approximately 60 days, with full competency targeted by the end of that period. The role supports referral management by monitoring hospital, health care, and referral portals; maintaining account-specific ownership; gathering and accurately communicating referral information; coordinating non-clinical aspects of referrals; and following up to support timely communication from referral through service initiation. It also helps referral partners, patients, and families understand BAYADA services and supports transitions from facility-based care to care at home. The position participates in market-development and community activities and represents BAYADA in hospitals, rehabilitation facilities, physician practices, and other referral environments. BAYADA’s mission is to help people have a safe home life with comfort, independence, and dignity. Its work is guided by The BAYADA Way: compassion, excellence, and reliability. BAYADA is an accredited, regulated, certified, and licensed home health care provider and an Equal Opportunity Employer.
