Home Health Nurse
About The Position
Pivot Home Health is hiring Nurses across Colorado to provide intermittent care through home visits in home-based and community settings. The role serves adults and geriatric patients, with flexible caseloads tailored to clinician preferences and no minimum or maximum caseload requirement. Clinicians choose their schedules and preferred travel radius, with no expectation to travel beyond their comfort zone. The position includes an iPad for electronic documentation, clinical leadership, 24/7 on-call support, mentorship, training, and professional development. Employment may be PRN, part-time, or full-time, with compensation of $47–$53 per visit.
Requirements
- Be licensed in Colorado as a Registered Nurse or Licensed Practical Nurse without stipulations.
- Hold current CPR certification.
- Hold current First Aid certification.
- Have a valid state driver's license, reliable transportation, and applicable car insurance if transporting patients.

Responsibilities
- Coordinate with a nursing partner to ensure complete coverage of care.
- Evaluate patients initially and periodically throughout care.
- Administer medications, injections, and IV infusions; perform wound, catheter, and ostomy care according to strict protocols.
- Coordinate care plans with physicians and other healthcare professionals and maintain accurate medical documentation in EMR/EHR systems such as Epic, Cerner, or Athenahealth.
- Educate patients and families about disease management, medication administration, nutrition, and self-care techniques.
- Monitor patient progress through diagnostic evaluations and physiological assessments and adjust care plans accordingly.
- Follow HIPAA regulations and infection control standards during patient interactions.
