Home Health Registered Nurse
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one skilled nursing care to patients in their homes, helping them recover and regain independence. You work with physicians and other clinicians to manage individualized care plans and support patients’ recovery. The role includes medication administration, wound care, patient assessments, monitoring patient conditions, educating patients and families, documenting care, coordinating with healthcare professionals, and overseeing Home Health Aides. This is a patient-facing role subject to Humana’s TB screening program and driver safety program.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.
- One year of nursing experience.

Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.
