About The Position
Mederi Caretenders, part of LHC Group and the Optum family of businesses, provides home health, hospice, and community-based services. The Home Health Scheduler manages patient referrals and visit schedules, assigning patient assessments and other physician-ordered visits through an online scheduling system. The role collaborates with the Team Leader to match clinicians’ experience and skills to patient needs. The position uses an automated scheduling system to maintain service calendars for episodic and per-visit customers, processes visit workflow changes, monitors referrals, and communicates with field staff to resolve visits that are late, pending, or incomplete. Requirements
- High school education or equivalent experience.
Nice To Haves
- At least 1 year of scheduling experience in a healthcare setting using an online scheduling system.
- Exceptional organizational, customer service, communication, and decision-making skills.
- Working knowledge of state and federal regulations governing OASIS visits, supervisory visits, and reassessment visits.
Responsibilities
- Use an automated scheduling system to maintain a calendar of services for episodic and per-visit customers.
- Process workflow for requested, scheduled, missed, rescheduled, reassigned, declined, and delivered visits.
- Monitor pending referrals daily and assign a licensed professional and case manager for all start-of-care visits.
- Communicate daily with field staff about visits in late, pending, or incomplete status and coordinate resolution as appropriate.
- Assign patient assessments and other physician-ordered visits using an online scheduling system.
- Collaborate with the Team Leader to identify clinicians whose experience and skills match patient needs.
