About The Position

Adoration Home Health is seeking a dedicated Home Health Scheduler to join its team in Clarksville, Tennessee. The Scheduler supports agency operations by coordinating patient visits, communicating patient information to staff, handling calls and visitors, and providing general administrative support. The role involves matching clinicians to patient assignments based on patient needs and location, clinician skills, ordered visit frequencies, and regulatory requirements. The position offers an independent work environment with team support.

Requirements

• High school graduate or GED.
• At least two years of administrative experience, preferably in home health.
• Proficiency with Microsoft Office, including Word, Excel, Outlook, and Internet Explorer, and ability to learn industry-specific software applications.
• Ability to work independently and in a team environment.
• Excellent oral, written, and interpersonal communication skills.

Responsibilities

• Greet visitors in a courteous, professional, and timely manner.
• Answer incoming calls from patients, staff, physician offices, and referral sources, and transfer callers to the appropriate person or department.
• Communicate patient information to appropriate staff to support a high level of care.
• Select an appropriate clinician for patient assignments based on patient needs, patient location, and clinician skills.
• Perform general office duties, including maintaining an adequate supply of items needed for staff and patient care.
• Support the clinical manager in managing patient schedules to ensure visits meet ordered frequencies and regulatory requirements.
