About The Position

Jet Health Inc / First Choice Home Health & Hospice is seeking a dedicated full-time Home Health Scheduler to join its home health team at the Boise, Idaho office. The role schedules patient and client services and coordinates with staff to support high-quality care in Boise and surrounding areas. This is not a remote position; the successful candidate will commute to the Boise office.

Requirements

Nice To Haves

• One year of experience as a home health or hospice scheduler is highly preferred.
• Experience with electronic medical records; HCHB is preferred.

Responsibilities

• Schedule patient/client services and document schedules appropriately and in a timely manner.
• Coordinate care with management and the patient/client interdisciplinary team.
• Communicate effectively with patients/clients, staff members, other health care professionals, and referral sources.
• Process reports in a timely manner to ensure compliance.
• Respond to agency and patient/client needs professionally and creatively.
• Promote the agency philosophy and mission by presenting a positive image to patients, families, physicians, and community agencies.
• Demonstrate commitment, professional growth, and competency.
• Maintain confidentiality while communicating effectively with patients/families, team members, and other health care professionals.
• Perform other duties as assigned.
