About The Position

Adoration Home Health is seeking a dedicated Home Health Scheduler to join its team in Nashville, Tennessee. The Scheduler provides administrative support to the agency, helps coordinate patient assignments and schedules, and supports communication among patients, staff, physician offices, and referral sources. The role calls for courteous service, strong communication skills, and the ability to work independently while collaborating with a team.

Requirements

• High school diploma or GED.
• At least two years of administrative experience; home health experience is preferred.
• Proficiency with Microsoft Office, including Word, Excel, and Outlook, and Internet Explorer, with the ability to learn industry-specific software applications.
• Ability to work independently and in a team environment.
• Excellent oral, written, and interpersonal communication skills.

Responsibilities

• Greet visitors courteously, professionally, and in a timely manner.
• Assist with incoming calls from patients, staff, physician offices, referral sources, and others, and transfer callers to the appropriate person or department.
• Communicate patient information to appropriate staff to support a high level of care.
• Select an appropriate clinician for each patient assignment based on patient needs, location, and clinician skills.
• Perform general office duties to support the agency, including maintaining adequate supplies for staff and patient care.
• Support the clinical manager in managing patient schedules so visits occur according to ordered frequencies and regulatory requirements.
