Home Health Nurse- LPN/ RN
About The Position
Centria Healthcare is hiring licensed RNs or LPNs to provide private-duty nursing in clients’ homes. The position described has an immediate need to care for pediatric patients from birth through age 21, delivering personalized, high-quality care in a comfortable home environment. Centria serves patients of all ages, including children with complex medical needs and adults recovering from severe traumatic injuries. Nurses provide one-on-one care and may deliver skilled services such as tracheostomy care, ventilator management when applicable, catheter care, wound care, medication administration, and bowel and bladder programs. The role also includes assistance with activities of daily living, monitoring the patient’s condition, emotional support, and coordination with families, caregivers, and medical professionals. The posting describes a specific day-shift position while noting that full-time and part-time shifts, including day, night, and weekend shifts, are available generally. Centria offers training, ongoing support, and opportunities to shadow experienced nurses. Pay is $28.00–$33.00 per hour, paid weekly.
Requirements
- Hold an active RN or LPN license in Michigan.
- Provide compassionate, patient-centered care.
- Have strong communication and teamwork skills.
Nice To Haves
- Experience in home health, private-duty nursing, or long-term care.
- For pediatric positions, experience with tracheostomy and ventilator care.
- Knowledge of paraplegia-related care, including skin integrity, mobility assistance, and respiratory support.
Responsibilities
- Provide skilled nursing care, including tracheostomy care, ventilator management when applicable, catheter care, wound care, medication administration, and bowel and bladder programs.
- Assist with activities of daily living, including repositioning, transfers, and hygiene.
- Monitor vital signs, symptoms, and overall well-being, and report concerns to the healthcare team.
- Offer emotional support and companionship to help maintain a positive and comfortable environment.
- Collaborate with family members, caregivers, and medical professionals to maintain continuity of care.
