About The Position

The Home Health Intake Nurse RN receives patient referral information from physicians, acute care settings, subacute and intermediate care facilities, and other sources, and determines whether patients are appropriate for home care services. The role works under the direction of the Manager of Clinical Operations and Director, Home Care Services. The position supports the intake process by interpreting referrals, coordinating admission scheduling with nursing and therapy leaders, entering referrals into the agency electronic medical record, and coordinating insurance verification. The nurse also maintains relationships with internal and external referral sources, provides professional customer service, attends required meetings and training, and completes annual employee requirements.

Requirements

Associate degree or diploma in nursing required.
Minimum of five years of nursing experience.
Registered Nurse with a valid or eligible Connecticut license required.
Excellent written and verbal skills.
Ability to work effectively with outside personnel from various healthcare settings and internal staff from all disciplines.
Ability to learn quickly, use critical thinking skills, and multitask.
Strong computer skills.

Nice To Haves

BSN in Nursing preferred.
At least three years of experience in home health care preferred.
Knowledge of insurance, managed care, PDGM, OASIS, and home care eligibility requirements.
Coding knowledge desirable.

Responsibilities

Receive agency referrals from all sources via EPIC Care Link, phone calls, or fax.
Interpret referrals for diagnosis clarity, specific service requests, and safety.
Coordinate with the nurse manager and/or therapy manager or supervisor as needed to schedule admissions.
Enter referrals into the agency EMR.
Coordinate with clinical support to verify insurance information.
May be requested to prepare monthly referral activity reports for the Director, Home Care Services.
Develop and maintain strong working relationships within YNHHS and with outside referral sources.
Exhibit excellent customer service, professionalism, problem solving, resource management, teamwork, and cooperation.
Attend required staff meetings, inservices, and other required meetings.
Complete all annual employee requirements in a timely manner.
