About The Position

Elara Caring is seeking a full-time Registered Nurse Clinical Team Manager in La Grange to lead and support field nurses delivering home health care. The role uses clinical judgment and a supportive leadership approach to help nurses grow, maintain clinical standards, address care challenges, and ensure patients receive appropriate care at home. The manager also supports quality improvement and compliance while helping the team meet productivity expectations without compromising care quality.

Requirements

• Current RN license.
• At least 3 years of active RN experience, including 1 year in home health.
• Knowledge of Medicare/Medicaid home care benefits, policies, and procedures.
• Strong clinical judgment, communication, and organizational skills.
• Commitment to coaching nurses and delivering patient-centered care.
• Valid driver’s license, insurance, and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 lbs.

Responsibilities

• Provide direction, coaching, and ongoing support to field nurses.
• Address clinical concerns and recommend appropriate interventions.
• Review nursing visits and documentation for accuracy and compliance with state, federal, and contractual requirements.
• Help nurses meet productivity expectations while maintaining quality care.
• Support Quality Assurance and Performance Improvement (QAPI) initiatives to strengthen clinical outcomes.
• Ensure timely documentation, follow-up, and communication with appropriate care partners and authorities.
