About The Position

Elara Caring is seeking an RN Clinical Team Manager to lead and support a team of home health field nurses. The manager provides clinical guidance, helps resolve patient-care challenges, supports nurses’ professional growth, and maintains standards for quality care and compliance. The role also involves supporting performance and quality-improvement efforts and ensuring timely documentation, follow-up, and communication with care partners and authorities.

Requirements

• Current RN license.
• At least 3 years of active RN experience, including 1 year in home health.
• Knowledge of Medicare/Medicaid home care benefits, policies, and procedures.
• Strong clinical judgment, communication, and organizational skills.
• Commitment to coaching nurses and delivering patient-centered care.
• Valid driver’s license, insurance, and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently, and to lift 50–100 lbs.

Responsibilities

• Provide direction, coaching, and ongoing support to field nurses.
• Address clinical concerns and recommend appropriate patient-care interventions.
• Review nursing visits and documentation for accuracy and compliance with state, federal, and contractual requirements.
• Help nurses meet productivity expectations while maintaining quality care.
• Support Quality Assurance and Performance Improvement (QAPI) initiatives to strengthen clinical outcomes.
• Ensure timely documentation, follow-up, and communication with appropriate care partners and authorities.
