About The Position

Southwoods Health is seeking a dynamic and qualified Home Health Clinical Manager. In this role, you will safely supervise and guide direct patient care in the home setting. Responsibilities include implementing plans of care, reviewing clinical documentation, and providing hands-on staff coaching and education.

Requirements

• Active Registered Nurse (RN) license in the State of Ohio.
• Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
• Advanced critical thinking, proactive problem-solving abilities, and a strong capacity to articulately document a patient's clinical picture.
• Proven leadership experience with the ability to delegate tasks appropriately to a multidisciplinary team.
• Exceptional verbal and written communication skills paired with a consistently professional demeanor.
• Excellent prioritization, organization, and time management skills.

Nice To Haves

• Bachelor of Science in Nursing (BSN) is highly preferred.

Responsibilities

• Act as a clinical leader to actively support positive patient outcomes and exceptional care experiences.
• Review and optimize care plans to meet patient needs safely, effectively, and efficiently.
• Manage a multidisciplinary team to ensure the accurate, safe, and compassionate delivery of home care.
• Monitor clinical documentation, perform regular audits, and implement corrective action plans to fix deficiencies.
• Assist with the onboarding, orientation, and continuous training of new clinical staff members.
• Ensure all home health processes maintain strict compliance with relevant regulatory agencies (e.g., OSHA, ODH, BOP, TJC).
• Drive cost containment initiatives by utilizing facility resources appropriately.
• Maintain absolute patient privacy and guarantee the strict confidentiality of all protected health information (PHI/HIPAA).
• Participate in facility training initiatives and perform other leadership duties as assigned.
