About The Position

Residential Home Health and Hospice is hiring a Certified Nursing Assistant to provide home health care to patients in their homes in the Glen Carbon area. The position involves delivering compassionate, patient-centered care in a hospice setting and collaborating with an interdisciplinary team focused on comfort, dignity, and quality of life. The CNA provides hands-on assistance with activities of daily living, supports patients and caregivers, and works independently while remaining connected to the care team. The role is field-based and requires travel to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state where the employee works.
• High school diploma or certificate.
• At least one year of healthcare experience.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting in collaboration with an interdisciplinary team.
• Assist patients with activities of daily living in their homes.
• Observe and report changes in patient condition to support safety and independence.
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks.
• Respond promptly to patient needs and communicate effectively with the care team.
• Maintain accurate, timely documentation in the EMR.
• Travel to patient homes within the assigned territory.
