About The Position

Transition Home Healthcare is hiring a Certified Nursing Assistant to provide home health and hospice care. The CNA works in coordination with the assigned nursing care team and helps deliver quality care and service to patients and their families. Coverage areas include North, Northeast, Northwest, and Southwest. The position is listed as PRN.

Requirements

• Hold a CNA license in the state of Texas.
• Have one year of home health experience.
• Be able to lift up to 50 pounds.
• Be able to work occasional weekends, holidays, and evenings.
• Have strong communication and people skills.

• Experience with HCHB EMR.

Responsibilities

• Provide exceptional care in coordination with the assigned nursing care team.
• Maintain patient records, including changes in doctor’s orders, diet, or personal living activities.
• Assist with meals, feeding, and basic activities of daily living.
• Deliver weekly reports to the case manager and review important changes in patient conditions.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with healthcare providers and other care team members.
