About The Position
Elara Caring is seeking a compassionate Certified Nurse Aide to provide home health care. Under the direction of a Registered Nurse, the aide delivers hands-on personal care, supports patients with daily activities and mobility, records vital signs, and follows individualized care plans. The role also involves monitoring and reporting changes in patient condition, maintaining a safe and comfortable home environment, reminding patients about medications, responding to emergencies, following safety and infection control guidelines, documenting care accurately, protecting patient privacy, and completing required training.
Requirements
- Current state Home Health Aide or Certified Nursing Aide certification.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
Responsibilities
- Provide hands-on personal care under the direction of a Registered Nurse.
- Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
- Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
- Help patients walk, transfer, and use mobility equipment safely.
- Follow each patient's individualized care plan.
- Monitor and report changes in a patient's condition to the care team.
- Help maintain a clean, safe, and comfortable home environment.
- Remind patients to take medications as directed.
- Respond appropriately to emergency situations and follow safety and infection control guidelines.
- Maintain accurate documentation, protect patient privacy, and participate in required training.
