About The Position
Residential Home Health and Hospice is hiring a Certified Nursing Assistant for a PRN home health position serving patients in their homes in the Glen Carbon, Illinois area. The role provides hands-on, patient-centered care, supports patients’ comfort, dignity, quality of life, safety, and independence, and involves collaboration with an interdisciplinary care team. The position is field-based and requires travel to patient homes within the assigned territory. Approximately 20% of the workday is spent driving to and from patient locations. Requirements
- Active Certified Nursing Assistant (CNA) certificate in the state where the employee works.
- High school diploma or certificate.
- At least one year of healthcare experience.
- Valid driver’s license.
- Ability to spend approximately 20% of the workday driving to and from patient locations.
Responsibilities
- Provide compassionate, patient-centered care in a hospice setting in collaboration with an interdisciplinary team.
- Assist patients with activities of daily living in their homes.
- Observe and report changes in patients’ conditions to support safety and independence.
- Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks.
- Respond promptly to patient needs and communicate effectively with the care team.
- Maintain accurate, timely documentation in the EMR.
- Work independently while remaining connected to the care team.
- Travel to patient homes within the assigned territory.
