About The Position

Transition Home Healthcare is hiring a Certified Nursing Assistant to provide home health care in the North, Northeast, and Northwest service areas. The CNA works in coordination with the assigned nursing care team to provide patient care, document clinical information, support patients and families, and communicate with healthcare providers and care team members. The available schedule is PRN.

Requirements

• CNA license in the state of Texas.
• One year of home health experience.
• Ability to lift up to 50 pounds.
• Ability to work occasional weekends, holidays, and evenings.
• Strong communication and people skills.

Nice To Haves

• Experience with HCHB EMR is preferred but not required.

Responsibilities

• Provide exceptional care in coordination with the assigned nursing care team.
• Maintain patient records, including changes in doctor’s orders, diet, or personal living activities.
• Help with patient meals, feeding, and basic activities of daily living.
• Deliver weekly reports to the case manager and review important changes in patient conditions.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with healthcare providers and other care team members.
