Home Care Registered Nurse-RN
About The Position
The Home Health Registered Nurse delivers skilled nursing care in patients’ homes. The role involves creating individualized care plans, administering medications, educating patients and families, and supporting recovery and independence. Nurses with home health experience and those transitioning from hospital, acute care, or skilled nursing settings are welcome. The position offers flexible, autonomous practice and one-to-one patient relationships. The role is based in West Hartford and follows a Monday–Friday, 8 a.m.–5 p.m. schedule, with a weekend rotation every four weeks and two holidays. There are no on-call requirements. The position uses the Epic EHR system for documentation and care coordination. Trinity Health Of New England At Home is a faith-based, not-for-profit home care, hospice, and palliative care organization. Its services include skilled nursing, physical, occupational, and speech therapy, and medical social work.
Requirements
- Graduate of an approved nursing education program.
- Registered Nurse licensure in the State of Connecticut.
- One year of experience as a professional acute care nurse.
- Current driver’s license and reliable transportation.
Nice To Haves
- Bachelor of Science in Nursing (BSN) preferred.
- Home care experience preferred but not required.
